Debra McGrath is a Clinical Nurse Specialist in Maternal Child Health and a Nurse Practitioner with extensive experience in the strategy of self-measured blood pressure as a tool to improve hypertension control and reduce cardiovascular risk in her role as the Special Projects Director for Population Health at the Health Federation of Philadelphia. Through her work on the Million Hearts project, Ms. McGrath has provided technical assistance to health centers implementing and maintaining SMBP programs. The assistance includes project and change management tools, education for the care teams, including the primary care provider staff, patient education materials, assistance in the selection and purchase of BP cuffs that are validated and self-reporting, development of new clinical forms for the EHR to assist in documentation and data collection for patients enrolled in the program, integration with a clinical data repository for electronic sharing of home BP readings and more recently guidance on how to include clinical pharmacists in the care team.
