Clinical Director, Georgia Primary Care Association

Dr. Jacobs is a board-certified family medicine physician and serves as the Medical Director for the Georgia Family Planning Systems (The Title X program) and the Georgia Association for Primary Health Care, which is the governing body for FQHCs in Georgia. She serves as a clinical advisor for NACHC Million Hearts Preventing Heart Attacks and Strokes in Primary Care project. And she is a member of NACHC’s Clinical Practice Committee. Dr. Jacobs currently is the interim Clinical Medical Director for Recovery Consultants of Atlanta. Dr. Jacobs is a scholar who earned an Associate Degree of Science in Industrial Chemistry from Ferris State University, Big Rapids, Michigan; a Bachelor of Science in Microbiology from Eastern Michigan University, Ypsilanti, Michigan; and her Medical of Doctorate Degree from Michigan State University College of Human Medicine, East Lansing, Michigan. She completed her residency in Family Medicine at Morehouse School of Medicine, Atlanta, Georgia where she served as chief resident for one year. She is a Fellow of the American Academy of Family Physicians.
