Yuriko de la Cruz, MPH, CPHQ (she/her/ella) is the Program Manager, Social Drivers of Health at the National Association of Community Health Centers. She currently partners with health centers to identify patient-level social needs and to use SDOH data (specifically PRAPARE) to inform upstream efforts that advances health equity. She is proficient in health equity, cross-sectoral community engagement and facilitation; continuous quality improvement; population health; and practice transformation. She is a board member of the following nonprofit organizations: Resurrected Community Development Corporation; Promise Neighborhoods of the Lehigh Valley; Academy of Communication in Healthcare; and Health Outreach Partners. Yuriko completed the Bachelor of Science in Public Health from West Chester University and the Master of Public Health from the University of North Carolina - Chapel Hill.
