Molly Volk, MHS, PCMH CCE is a Practice Coach at OCHIN and currently supports a range of care transformation projects, including social determinants of health screening and referral-making, clinician burden and wellness, and virtual care. Ms. Volk is passionate about transforming health care to better meet the needs of patients everywhere. In her role as Practice Coach, Ms. Volk provides practice transformation support to OCHIN’s members and customers to help them establish and reach clinical, operational, and quality improvement goals; adopt best practices; implement innovations; and improve patient outcomes. Ms. Volk is an empathic inquisitor who partners with clinicians and quality leaders to explore and operationalize new knowledge in pursuit of the Quintuple Aim. Before OCHIN, Ms. Volk supported Idaho’s Statewide Healthcare Innovation Plan (SHIP) in various capacities including coordination and evaluation roles. The SHIP was a Center for Medicare and Medicaid Innovation-funded approach to reform Idaho’s healthcare delivery system to one that values improved health outcomes through a patient-centered focus. Ms. Volk enjoyed her time traveling around the beautiful state of Idaho and learning from primary care teams about their experiences transforming into patient-centered medical homes. Ms. Volk splits her time between an island in Alaska and an island in Washington state where she spends most of her time outdoors, rain or shine. Her favorite hobbies include hiking with her puppy, exploring with her partner, and cooking for loved ones.
