Practice Transformation Manager, OCHIN

Ann Romer, MS, is the Manager of the Practice Transformation team at OCHIN, overseeing a team of consultants and coaches who support a national network of health care sites in the OCHIN collaborative. She has been the principal or supervising practice coach supporting members with adoption of team-based care processes and EHR-integrated tools through tailored, scalable, virtual and in-person support, as well as overseeing the development of a medical home consultation program tailored to meet the documentation and reporting needs for OCHIN's 200+ ambulatory member clinics. Prior to joining OCHIN, she served as practice facilitator for the Oregon region of CMMI’s CPC initiative, supporting a 67-clinic cohort to transform to an advanced primary care model. As manager, Ms. Romer has led the Practice Transformation team through over two dozen multi-site, multi-year transformation projects, with strategic focus on addressing clinician wellness and advancing equity. She is motivated by service to the public good, formerly serving as a Peace Corps Volunteer in the Philippines, where she first developed her interest in applying the art and science of conflict management to support collaborative relationships. Ms. Romer holds a MS degree in Conflict Resolution, and since 2012 has served on the board of Unite Oregon to support justice advocacy and organizing across the State. She lives in Portland with her daughter, husband, dog and cat.
