LuAnn Kimker RN MSN, PCMH CCE is the Vice President of Clinical Innovation at Azara Healthcare. In a healthcare environment that is constantly changing she is passionate about using data to drive change and improve the systems that impact patient outcomes. LuAnn works with primary care network and communityhealth center leaders to identify the tools needed to build and improve their population health programs. She draws extensively on her experience managing primary care and specialty practices operations, quality improvement programs and developing sustainable Patient Centered Medical Home programs to develop data strategies to help improve patient outcomes. LuAnn encourages the integration of various data sourcesto more efficiently manage the data, systems and people needed for population health program.
