Registered Nurse Care Manager, Esperanza Health Center

Nate Pena has been a registered nurse at Esperanza Health Centers for four years. He is involved in creating and developing projects to improve patient health outcomes, including an RN led diabetes titration program and the use of continuous glucose monitoring for underserved patients with type 2 diabetes. Since the start of the pandemic, Nate’s work has expanded to include triaging patients with suspected COVID-19 and a new role as Infection Control Champion as sites reopen. Nate looks forward to seeing how nursing will continue to evolve across all health care settings in response to challenges the future may bring.
