Deena has over ten years of healthcare and public health experience and is the Program Lead for the Los Angeles Practice Transformation Network, a $16M CMS-sponsored grant initiative to transform clinical practice and achieve large-scale health transformation. She leads the team to deliver coaching services to bring 800 targeted providers through five phases of transformation and ready the practices for value-based reimbursement while demonstrating significant improvements in care for patients with diabetes and/or depression. Deena oversees project compliance and program goals for 23 practices and manages a team of 10 practice transformation coaches. Deena is also an assigned coach directly supporting two PCMH recognized FQHCs under the program. A Patient-Centered Medical Home Certified Content Expert, Deena aligns her practice transformation work with PCMH recognition.
