Yuriko de la Cruz MPH

Yuriko de la Cruz, MPH, CPHQ (she/her/ella) is the Social Determinants of Health Manager at the National Association of Community Health Centers. She partners with health centers to identify patient-level social needs and to use SDOH data to inform upstream community efforts that advances health equity. She is proficient in addressing SDOH, cross-sectoral community engagement; training and technical assistance; continuous quality improvement; and population health. She serves as a board member of two organizations in Allentown, PA: Resurrected Community Development Corporation and Promise Neighborhoods of the Lehigh Valley and a national organization: Academy of Communication in Healthcare. Yuriko completed the Bachelor of Science in Public Health from West Chester University and the Master of Public Health from the University of North Carolina - Chapel Hill.
