Social Services Triage Representative, Start Community Health Center
Start Corporation’s Social Services Triage Representative, Heather Rogers, is a patient navigator for the Promoting and Integration of Primary and Behavioral Healthcare program as well as triage representative for multiple social workers with four years’ experience in case management and community resource information. As a patient navigator with the PIPBHC program and in conjunction with PRAPARE, she collects information regarding a client’s social determinants of health as well as their medical history, social history, and preventive screening information. Internal and/or external resources are given to the client based on the client’s need. As a social service triage representative, she also works closely with front office staff to assure that all demographic information is completed and correct.
