The Program of All-Inclusive Care for the Elderly (PACE) provides comprehensive medical and social services to eligible individuals who live in a community. PACE benefits include, but are not limited to, all Medicaid and Medicare covered services: adult day care, dentistry, emergency services, home care, hospital care, nutrition counseling, primary care, prescription management, and transportation services between services. Since 2019, NACHC, Capital Link, the National PACE Association, and the Galway Group have offered resources and support to health centers through the PACE@CHCs initiative. This has helped to inform and educate health centers nationwide to explore launching PACE at their centers. This dual didactic and experiential learning opportunity will allow participants to hear from national experts and health centers about the program and visit PACE sites through San Ysidro Health (transportation provided).
