Catherine is the VP Product, Transitions of Care and High Risk at Aledade, Inc., a primary care-focused ACO enablement organization serving over 600,000 beneficiaries across 26 states. She leads a cross-functional team of innovators who are tasked with building out Aledade's platform and solutions to support PCPs across the nation (including many FQHCs!) in value-based care success. She is responsible for developing the Aledade strategy to leverage data, technology, and point-of-care workflows to achieve high-value care for the most vulnerable populations these practices serve.
