Michelle Proser is Director of Research at NACHC where she has 15+ years of experience applying research and analysis to uncover and explain health centers’ role in the health care system, as well as empowering health centers in their efforts to improve population health. Her research focus includes identifying the mechanisms that drive health center value and monitoring the impact of policy and health system changes on health centers, their patients, and their communities. Michelle also leads a nationally-recognized, multi-partner effort to help health centers document the extent to which patients are socially complex, and use that information to accelerate health improvements and health care transformation. Known as the Protocol for Responding to and Assessing Patients Assets, Risks, and Experiences (PRAPARE), this screening tool and companion action toolkit is now commonly used across health centers and other providers, and has accelerated health improvement at the patient and community levels.
