The Health Center Program Requirements are a critical foundation for a health center’s success, and HRSA’s Site Visit Protocol has changed the way operational site visits (OSVs) are conducted and how HRSA assesses compliance with program requirements.  

This session will share lessons learned and best practices at all phases of the operational site visit process. Participants will leave this session with innovative strategies and insight into how high-performing health centers use the OSV to engage in a continuous process of not just program compliance, but performance improvement and operational excellence.

Some 2.7 million LGBT people are age 50 or older; this number will double by 2060. LGBT older adults experience health disparities in cardiovascular health, cancer, disability, and behavioral health. They often struggle to access culturally competent and affirming health care and elder services. Many also experience social isolation and aging alone, and discrimination in housing. This session will focus on policy approaches to improve health care and elder care for LGBT older adults and what health centers can do to better serve older LGBT patients.

As social determinants of health (SDH) become more recognized as significant impacts to healthy living, increasing numbers of state payers and stakeholders are getting involved. How do you ensure that your SDH work aligns with and informs other initiatives that are occurring in your state? In this session, PCA and HCCN representatives will highlight their efforts using their work on PRAPARE to inform SDH policy and practice happening in their states. In particular, presenters will discuss different methods and strategies to showcase their specific PRAPARE experiences and findings while engaging in state-level transformation discussions with payers, funders, and policymakers. This session will detail how states can use these strategies to lay the groundwork and drive next steps in social determinants of health policy.
