Aledade is a partner for FQHCs embarking on their accountable care journey through support of Accountable Care Organizations. As an organization focused on primary care providers, Aledade supports and empowers FQHCs through a blend of technology and health care to remain financially viable in an ever-changing payment environment.

Based on their experience working with FQHCs, Aledade sees a need for education, a focus on annual wellness visits, and the use of data as keys to success. This document provides examples from their work on how those strategies can be implemented.
