Description

This publication is part of a series of case studies that highlight innovative models for providing enabling services to help reduce barriers to care and address social determinants of health.

In collaboration with community partners throughout the state, the North Carolina Farmworker Health Program (NCFHP) provides and leverages a wide range of enabling and health care services to migrant and seasonal farm-workers and their families. This case study features Native Health, an Urban Indian Health Center that provides a suite of enabling services specifically designed to address food insecurity and promote healthy food choices.
