Part two of this webinar series will focus on telehealth and other alternative ways to engage patients, such as community health workers and integrated care managers. Health centers have a tremendous opportunity to leverage the relationship and trust built with their patients. Large corporations like Amazon, Apple, CVS Caremark, and others appear to have an increased interest in the health care market and may focus on the safety net population. Considering the amount of access they all have to our patients through purchase patterns, prescriptions, and in some cases, care delivery, this paradigm shift may come sooner than we think.Register today to hear how some CHCs have been using Telehealth to expand their reach into the living rooms of their patients.

This new delivery model is used for specialty and primary care where access has historically been challenging. In addition, some practices have also coupled care management or community health worker strategies with telehealth to provide more support, education, coaching and transportation for their patients.With better business intelligence tools, practices are using population health tools like registries, visit planning, risk stratification and other kinds of reports to help focus the efforts of these interventions and staff on the most vulnerable populations. In addition, the collection and use of social determinants of health have offered another lens for understanding and meeting the needs of patients. Primary care associations, health center-controlled networks, and networks often play a role in seeing and advocating for the improvement of shared business intelligence tools to support these new areas across their members. Join us to learn some of the keys for success, results, and lessons learned about alternative patient engagement strategies and the use population health tools to assist with targeted focus and outreach.
