Shannon Bafaro, MPH, joined the Missouri Primary Care Association in June 2014 and is a member of the Missouri Center for Healthcare Quality Team. She serves as the Director of Value Based Care, leading the state’s Health Center Control Network and the Missouri Quality Improvement Network (MOQuIN), while training on the optimization of population health analytics. Shannon received her Master’s in Public Health in 2015 from the University of Missouri. Her areas of focus include elevating collaboration amongst care teams, optimizing the use of tools and data to strengthen both clinical and population health efforts, and supporting organizations in understanding and adopting workflows and processes to support transitions to value based care. Prior to the MPCA, Shannon worked in quality and operations at a Federally Qualified Health Center in Chicago, Illinois, where she gained experience in launching organization-wide team based care, creating actionable dashboards and reporting, and integrating the Patient Centered Medical Home (PCMH) culture throughout the organization.
