Diane Fisher, BSN, serves as Chronic Disease Telehealth Nurse for Berks Community Health Center in Reading, PA, focusing on addressing the epidemic of hypertension in the underserved population of Berks County. As part of a team with Dr. Jasmine Mencia, she helped develop a comprehensive Self-Measured Blood Pressure Monitoring Program that addresses all components of hypertension. Ms. Fisher holds a BS in Nursing from Thomas Jefferson University. She has 26 years of nursing experience and has worked with a wide range of age groups and disease processes both in inpatient and community-based environments. She spent much of those years practicing in outpatient pediatrics focusing on childhood obesity, hoping to help prevent adult-onset hypertension and diabetes. She is currently pursuing a Master’s in Community Nursing.
