Alexis Dellogono, PharmD, BCACP
Alexis is a graduate of the University of Rhode Island where she received her Doctor of Pharmacy (PharmD) degree. She completed her post graduate year one residency training at the Holyoke Health Center (HHC) and was then hired as a full time clinical pharmacist at their sister site, Chicopee Health Center. In this role, Alexis provides a variety of clinical services including medication therapy management (MTM), transitions of care (TOC) and drug therapy management for hypertension, type 2 diabetes, and smoking cessation via collaborative practice agreement. She is also responsible for coordinating the pharmacist-led immunization services and corresponding grant work in conjunction with NACHC and other partners. Alexis is a passionate advocate for the underserved patient population at HHC. She also enjoys training pharmacy students and residents and serves as a preceptor for local colleges of pharmacy and the HHC pharmacy residency program.
